Whether you're a new patient or current member of our patient family, some of the information here may be of use to you. The standard forms we have in our office are available for download with the links below. If you're a new patient feel free to download them and print them out prior to your office visit. You can fill them out at your leisure at home and bring them in with you.
Methods
of Payment
We request payment at the time of service; unless other
arrangements have been made in advance.
We accept cash, check, Master Card, Discover, American Express
and Visa. We offer multiple financing options to
meet your individualized needs. Please visit with our financial
coordinator, Jen, she will be happy to assist you in determining
the best option for you.
We participate with all dental insurances and are currently
a preferred provider for Delta Dental. If you call us with your
dental insurance information, we will be happy to determine what
your plan will cover.
Cancellation Policy
Our patient's time is important to us. Scheduling appointments in advance is one of the best ways to ensure that our office can make the most of the time available to you. In order to best serve our patients, we ask that you kindly give 24 hours notice for any cancellations.
